Unverified or conflicting information

  • Number of capture views — PRD (docs/Prd …md:5,24) and systemflow.md:7-12 say 5 views; code and ADR 0008 say 3. Code behaviour recorded; OPEN-005.
  • Image storage — PRD names Cloudinary; code stores to Cloudflare R2. OPEN-005.
  • AI provider — PRD names Google Gemini directly; code calls OpenRouter (Gemini 2.5 Flash) and Groq. OPEN-005.
  • Offline access — ADR 0003/0004 describe a signed Access Lease; code uses a phone-side timer and offlineMinutesAllowed. OPEN-018.
  • ADR 0001 says the mobile app lives in a separate repository; it now lives in this repository. OPEN-013.
  • Hosting — Vercel configuration and a Render host are both present; which serves production is not recorded. OPEN-022.
  • Deployed version — on 2026-10-25 the production server lacked routes that exist on app_dev (/api/user/offline-limit-exceeded, /api/admin/doctors/[clerkId]/approve). OPEN-022.
  • Proposed, not decided — intended-use wording (IRS §1, CHR §3), decision rights and RACI (STK §2–4), the feasibility recommendation (FEA §9), objectives and milestones (CHR §4, §7). Each is labelled proposed and has an owner.
  • The scan's "Sensitive files" section reported none; manual checks found two committed credential files and nine committed database backups (OPEN-007, OPEN-008). Their contents were not opened.

Traceability gaps

none (sdlc.py trace: no IDs referenced without a definition; no REQ IDs yet — expected in 02-Requirements).

Stage gate readiness

  • Project ID is registered and the brief is complete — not met: PRJ-001 allocated in the brief, not registered in a tracker; sponsor not named (OPEN-001).
  • Feasibility recommendation and initiation risk screen are reviewed — not met: both drafted (FEA draft recommendation: conditional GO for OPT-002), neither reviewed.
  • Regulatory and privacy hold points are either cleared or scheduled with owners — partly met: all 8 hold points are listed with owners, gates and proposed dates (IRS §7, CHR §8); none is cleared.
  • Charter is approved by the sponsor with budget ceiling and success criteria — not met: charter drafted; sponsor, budget and targets are open (OPEN-001, OPEN-003, OPEN-020).
  • Kickoff is held and its decisions and actions are recorded — not met: kickoff never recorded; continuation kickoff proposed (OPEN-024, ACT-001).
  • Every record above is complete, or has an approved written disposition — needs human decision: all 7 drafted with owned gaps; no dispositions approved.
  • Stage-completion checklist completed by a named independent reviewer — not met (OPEN-001).

Suggested next step

Close OPEN-007 and OPEN-008 now; name the Sponsor, Product Owner, Tech Lead and Security & Privacy Officer (OPEN-001); have the Sponsor review the charter and feasibility recommendation; then run sdlc-02-requirements.
